Our services have expanded from moving individuals living in institutions to family settings into supporting adults and children with highly diverse support needs in a wide variety of homes and community-based settings.


Service Coordination is a case management service that assists individuals eligible for Medicaid in gaining and coordinating access to necessary care and services appropriate to the needs of the individual.



  • Must be at least 18 years of age
  • Bachelors Degree in a Human Service Field
  • Must have 12 months experience working with individuals with developmental disabilities
  • Have their own reliable transportation, a valid drivers license and car insurance
  • Must be able to pass State and Federal Background Check through the Department of Health and Welfare


  • Two (2) years' supervisor experience
  • Current First Aid/CPR Certification


  • Develop, implement, and revise participant plans, as necessary
  • Collaborate with participant, families, and other agencies to ensure services are provided
  • Maintain contact with families and participants, both via phone and face to face as required
  • Coordinate with the participant to obtain crisis resolution, as needed
  • Adhere to the rules of all governing agencies, including IDAPA
  • Provide assistance to people receiving support through the implementation of the RISE mission, vision, and values
  • Demonstrate respect and promote choice
  • Promote the rights of people receiving support and contribute to an environment that is free of abuse, neglect and or exploitation
  • Support people in achieving outcomes identified in the persons plan; this includes keeping required documentation and any prescribed or related task(s)
