Job Description
RISE Early Intervention is a growing organization that provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them to build on their child's strengths, and to enhance and promote the child's growth, development and learning skills in their natural environment. RISE EI provides support to families of children birth to three years of age with disabilities or special developmental needs by assisting with learning developmental skills such as motor and language, facilitate engagement in activities, routines and events of everyday life.
PURPOSE: Service Coordination is a case management activity which assists individuals eligible for Medicaid in gaining and coordinating access to necessary care and services appropriate to the needs of the individual.
ESSENTIAL JOB FUNCTIONS:
Support the Mission, Vision, and Values of RISE Early Intervention
Plan Assessment and Periodic Reassessment.
Activities that are required to determine participant's needs by the development of an assessment of participant strengths and needs and periodic reassessment as described in Health & Welfare rules.
Take a participant's history
Identify the participants strengths and needs and complete related documentation
Gather information from other sources such as family members, medical providers, social workers, and educators to form a complete assessment of the participant.
Development of the Plan
Develop and revise the plan and address medical. Psychiatric, social, early intervention, educational and other services needed by the participant. It will be updated at least annually.
Referral and Related Activities-Linking
Link the participant with medical, psychiatric, social, intervention, educational providers or other programs and services that are capable of providing needed services to address identified needs and achieve goals specified in the plan. This will include natural supports and unpaid services.
Monitor and follow up with contacts that are necessary to ensure the plan is implemented and adequately addresses the participant's needs. These activities may be with the participant, family members, providers, or other entities or individuals and conducted as frequently as necessary. These activities must include a face to face contact with the participant every 90 days (Participant and team member) to determine that the following conditions are met.
Services are provided according to the plan
Service in the plan are adequate
Changes or adjustments are made if needed
Ensure proper maintenance of child files, required documentation, the ITP automated system, and RISE tracking tools
Coordinating
Coordinate, attend and facilitate assessments, IFSP Meetings (Initial, 6 month and annual), and transition planning
Participate as an integral part of the Multi-Disciplinary Team
Schedule initial meeting with families, and conduct screenings using tools approved by Idaho Health & Welfare; ensure timely delivery of services
Professional written and verbal communication with children, families, team members, Idaho Health & Welfare personnel, other professional providers and community members; coordinate and monitor delivery of services among multiple agencies, medical and health providers
Ability to build relationships and to communicate with multiple customers including the families receiving support, service professionals, foster parents, advisors, natural parents, state personnel and others.
Ensure that all communication and interaction with consumers, families, children, employees, contractors, and other stakeholders is conducted in a culturally competent manner
Local travel to homes throughout Ada county and/or Canyon county
Crisis assistance
Assist a participant to access community resources in order to resolve a crisis.
Health & Welfare and agency rule compliance
Follow all ID APA rules and agency policies and procedures while providing services to participants.
Initial consultations with new families
EQUIPMENT NORMALLY USED:
Computer, copy machine, telephone, fax machine, and other office equipment as necessary
PHYSICAL AND MENTAL REQUIREMENTS:
Ability to quickly respond to health, safety, and emergency situations
Ability to work under stressful conditions that may include dealing situations while handling multiple work assignments
Ability to have appropriate interactions and maintain professionalism with RISE Services, Inc. employees, customers, and people in the community
MINIMUM QUALIFICATIONS:
At least 21 years old
Ability to pass a criminal history background check
Bachelors degree in human service field
Strong verbal and communication skills
